Thanks to Bill Bolloten, who told me I cannot send attachments to this mailing list, and gave me an alternative. If you go to the address below, the file is there and can be downloaded - I have just tried it. You have 7 days.

If you still have a problem, I will send it to you individually, but copying and pasting about 50 addresses was really a bit daunting! I am hoping I can send this now - my previous attempts met with failure.
