[eal-bilingual] peer/self evaluation format for eal teachers

Hi
Does anyone know of a format that could be used to structure peer/self evaluation for eal teachers? I'm looking for something that would be useful for:
1. Observation of/reflection on practice in the classroom working with pupils 
2. Self evaluation of the full range of skills & knowledge - such as you can quite easily find in a generic format for teachers meeting the standard for full registration etc, but relating to the specific set of skills//knowledge of eal teachers.
