Perhaps it might be helpful first of all to have a time stamp on all pages (including translations) indicating the last time the content was updated. Then, if someone makes significant changes to a page, perhaps a changelog for translators (in the spirit of wiki recent changes or something) somewhere on the site (a link on the how-to translate page ?) that translators could consult on a regular basis to determine if they need to update their translations ?
