  What you describe sounds like what we do.  I write them myself using the data I have and collating information from outside agencies.  I will allow up to 2 sides of A4 but not more and preferably much less.
  We do 
  Photo
  Description of area of need.
  KS2 SATS or KS3 SATs.
  CAT test scores from Year 7 and any comments on discrepancies.
  Latest reading and/or spelling ages if relevant.
  Any other relevant test scores eg IQ tests.
  Advice on management, strategies etc from outside agencies.
  Access arrangements.
  I expect subject teachers to decide on their subject specific targets.  I monitor those.
   
  What did Ofsted want?  Did they want a detailed IEP for each subject for each pupil?

Ours start with a pen portrait of the pupil written in conjunction with the pupil and sometimes the parents too. We list their academic achievements and levels, obviously targets and there is a space for strategies. There is a space for evaluation and for pupil comments.

We do not like any of the programmes we have seen. We feel they are too impersonal. However I could add in anything anyone else thinks works well.
