  I only supervise up to GCSE level but I think the instructions we use are general.  Basically, it means not giving an advantage to the person being read to.
   
  As Sharon said, there are rules about who can be a reader and/or a scribe.  See section 2.2.8 of http://www.jcq.org.uk/access_arrangements/  for who can be a reader and 2.3.8 for who can be a scribe.  The basic rule is that the subject teacher must not do it.  Like many, we also say that the TA who usually works with the pupil in that lesson may not do it.  We use TAs as readers and scribes and we specifically train them in what to do.
  If the question asks for something like 'write 1234 in words' (unlikely at AS level I know but it has cropped up in KS3 SATs) you have to say 'write one two three four in words' so they dictate one thousand two hundred and thirty four' because if you say it in words you give them an advantage.  If you are signing for a deaf pupil, you have to finger spell words like perimeter and not use a genreic sign as it could give an advantage.
  We read each question all through so if the formula is in the question, we read it.  If the formula is on the paper at the beginning, we read it on request because the candidate needs to decide that they need the forumula and remember it is there.
  You can only read questions again on request.
   
  I think the hardest part of all of the access arrangements process is being a scribe and having to write down right and wrong answers with an equally straight face!
