ots of lesson plans and actvities for primary age range, but many would be useful as they explore basic misunderstandings which are at the root of students difficulties only covers number.
Rigby Heinemann used to produce a great set of books called Bridges to Numeracy which broke down the Primary key objectives into small steps. Sadly out of print, although if you can locate a copy they will allow photocopying...
Beam do fantatsic maths resources, especially their word problem big books and sheets.
I work a lot with Y6 who are at level 2/3 and we do lots on time, money, play cards, weighing and measuring to make things...life skills - rather than specific maths activities.
