A sad Military History buff adds:
Doolally...yes...'twas the famous lock hospital
for the military during the Raj.

'East Ham'.... would guess, as J says, cockney
rhyming slang...but not for the Indian doolally rather the English
Mad House. ie The Bethlehem Hospital shortened in
English to 'Bedlam'.
