My provision maps are very simple, just started this year. An Excel document
into which I drop the SEN register, sorted into year groups. ( I have been
quite rigorous about this - only include students that have current
provision - but this will include some BESD where the Head of year has lots
of input without much involvement of the learning support dept.)

Across the top list all the provision that is available - from in-class
suppport, through withdrawal for additional literacy, to modified timetable
/ additional work experience etc. It's usually a bit of a squash to fit all
the columns in. Last column has "Other agencies". I haven't got beyond hand
ticking the appropriate columns for each student, but intend to get an admin
asst to enter them on the computer - with some colour coding to indicate
current or past provision. Very useful exercise, highlighted one or two that
we thought had withdrawal when they didn't, etc!
