[senco-forum] re-integration

In my present position I help to re-integrate excluded KS3 pupils back into mainstream provision. I am looking for like a set of worksheets which a Learning Mentor could use to prepare a pupil for re-integration eg making new friends, bullying issues, finding help etc.
Does anyone know where I may be able to find such a programme?
