Hi Janice
  Extra time is as well as rest breaks, I'm sure.
  From the booklet:

  6.1.5 Supervised rest breaks
  Supervised rest breaks may be considered as an alternative or addition to
  any request for extra time. Centres may permit a candidate to take
  supervised rest breaks for medical/psychological reasons. The timing of
  the examination should be stopped and re-started when the candidate is
  ready to continue. If the candidate needs to leave the examination room,
  an Invigilator must accompany the candidate. The timing of these breaks
  depends on the nature of the medical condition. Centres should ensure
  that medical advice is sought in cases of serious illness, and that the
  candidate is fit to take the examination. There will be some constraints on
  timing if the candidate has more than one examination to take each day.
  The supervised rest break is not included in any extra time allowance.
