I have a book,  "Standard Reading Tests" by J C Daniels and Hunter Diack
(Chatto & Windus)  which was the "bible" in our region in the 70s and
80s.  I still find it excellent.  It contains a series of tests on 
various skills involved in the reading process as well as one for 
overall reading skill and another for comprehension.  I found each of 
the tests easy to administer and relatively quick.

The one on visual discrimination  and orientation is simpler than the 
one you describe but I found it did pick up kids who had difficulty with 
these. (I have always been worried about offering kids lots of near-miss 
words to choose between-  seems to me to be likely to reinforce failure, 
or at least to be discouraging, if you see what I mean?)

I see Amazon has copies for about £5.  Worth a look.
