We had a similar anxiety from a pupil last year and three and six  years
ago. One of the things that helped was for the TA who was going to be
his chief support to visit him in the primary school on several
occasions and then for him to come for 3 or 4 additional visits. 

For the first two visits he came and left between lessons so he did not
see 2000 pupils moving. On this occasion he was shown round by two
former pupils of his primary school with TA and other adults following.
Photos were taken of key places and people on one visit and he sat in
the learning support base where pupils chatted and showed him their
contact books and dinner cards.  On another visit he went to "break" in
the dining room with two friends, at first before break time and another
time when break was happening and the dining hall was full. We did this
visit when Year 11 were on study leave so it cut the numbers down and I
think the first one was when Year 10 were on Work Experience.

Meeting the HOY and FT to be was also helpful and seeing where his form
room would be. We have very helpful "Pupil Services" staff who are
always introduced as the people who will help you if you get lost, lose
your timetable anything!  They are always smiley and encouraging too. 

We usually give the pupil a contact book and a current Year 7 timetable
so they can work out how to follow/use them and a visual timetable if
necessary.

A lad in Year 6 with a statement for Asperger's syndrome came to see me
on the open evening - he'll be coming to us next september but is
alwready worrying about it and it is affeting his behaviour at primary
school.
I will ask the EP etc but wonder if anyone out there had experience of
how to assist transition for such a lad. 
He has 20+ hours of TA support apparently including playtimes.
