A CAF is a common assessment framework form that you should be able to download possibly from the DFES website or teachernet. It is a tricky and length document which is meant to involve multi agencies so there is plenty of intervention to help a child. However, the parent owns the CAF at all times so no signature or permission means no intervention. Unless you can get Fraser consent from the child you are stuck and getting Fraser consent is iffy. 
For info a local school ran the CAF as a pilot they managed to fill in 3 all year due to the complexity and length of time they take!
