When taking children out on coaches etc, we as a school were expected to do a risk assessment that included the journey on the coach, with all of the things that you have mentioned. If we went to a specific building - theatre or some such, they were expected to have a risk assessment already in place, so we did not cover that part. I do not recall ever requesting one from a coach company, but perhaps regulations have changed in the six months since I retired!
