Dave, not using it but have looked at it;
seems to be the following categories where the teacher
judges themselves at one of 5 levels for each, with
examples to help them.
1a. My ICT skills
1b. My personal use of ICT outside of my role in
school
1c. My use of ICT for professional duties outside of
the classroom
1d. Confidence in my ability to use ICT
2a. Identifying opportunities to use ICT in my
teaching
2b. Planning the use of ICT in my teaching
2c. Evaluating ICT resources
2d. Making appropriate use of ICT in my teaching
2e. Understanding how ICT can improve my teaching
2f. Understanding how ICT can improve/support/enhance
pupils' learning
2g. Assessing pupils ICT capability
3a. My attitude towards using ICT in the classroom
3b. Reflecting on my practice using ICT in the
classroom
3c. Supporting others in their use of ICT for teaching
and learning

it's quite interesting, if a little repetitive. 
I guess it's designed to make the teacher think what
they do, what others do, and what they would like to
do. Not sure how useful it would be to audit a staff
with the aim of providing specific group training? 
have you used it with staff?
