The problems are that although the little digital recorders work well they 
have to record a trained voice in the usual conditions, so recording a 
teacher in a normal class situation is not going to work.  It is possible 
to record the lesson, replay it and echo the teacher's words in to dragon 
dictate - there are services for deaf people that use this method.

There is a program called Azam that can audio bookmark recordings so that 
the points in a talk or meeting can be access easily.  Free to people with 
disabilities the last I heard.

We have also been using the Digimemo note taker which can convert 
handwriting to text.  Useful as well because the student can see what the 
assistant is writing and be more involved.
