I think the ACE Centre in Oxford are currently running a project looking
at eye-gaze technology.  That would seem to tick a few boxes for you.

Hi I have a new students coming to my clas in september he has no head
movement but he can eye point. Does anybody know of any devices that
could be used to generate a cause and effect response? If
any of you are near Oxford it would be really good to come and see the
technology being used.
