The best (as in accurate and easiest to use) system I have had experience 
with is "ERICA" http://www.eyeresponse.com/Disabilities/ 

I wrote about it 18 months ago

And have seen it in use a few times since. But, it sure is not inexpensive. 
Still, it works beautifully, and getting it "configured" takes seconds.
